Just recently I was working with the 'titles' table of a large  
organisation's contact database. It contained several hundred  
permutations of 'Viscountess', 'Air Marshal' and 'Her Majesty', along  
with the correct form of address for mail-merge purposes for each one.

The one that puzzled me was the title 'The', who should apparently be  
addressed as "Dear [surname]". Eventually I came to the conclusion  
that it was a special case for 'Fonz', 'Edge' and 'Dalai Lama'.
