I've used Rails Machine for over three years and they have been  
fantastic.  Their support is speedy and effective and they have pulled  
me out of every single hole I've dug for myself in that time.

They offer managed hosting, though I haven't tried it (I'm on an  
Original Machine account, which means I administer my VPSs myself).   
Based on my experience with RM so far, I would expect it to be a first  
class service.
